12/18/2023 EnchantaVaughn Approved QMHPA held an individual session with Ms. Cosby. Upon arrival at Ms. Cosby’ home, Ms. Cosby presented with positive affect, evident by her facial expression and body language. QMHPA informed Ms. Cosby that this session was going to focus on improving reduce depressive symptoms, anxiety and resolve causes leading to normalization of emotional state.. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by assessing Ms. Cosby’ emotional well-being. QMHPA questioned Ms. Cosby about her triggers and stressors. QMHPA explained to Ms. Cosby different exercises that are useful to help manage her triggers. This conducted a mental skill building activity that guided Ms. Cosby with arm/wrist rotations and finger movements. QMHPA taught Ms. Cosby different techniques to cope with her depression. QMHPA taught Ms. Cosby about situations that cause her to be depressed. QMHPA encouraged Ms. Cosby display positive emotions and think positive. QMHPA explained to Ms. Cosby holding your emotions in will not help you have to let out your feelings at. Ms. Cosby actively participated in the session. While exploring Ms. Cosby’ emotional wellbeing, Ms. Cosby stated to QMHPA that she was in a positive mood. During the skills building activity, Ms. Cosby discussed with QMHPA that she has been a little depressed with some things that have been going on in her life; however, she declined to discuss them with QMHPA. Ms. Cosby discussed with QMHPA that she is at the point where she sometimes just feel like giving in the towel. Ms. Cosby was able to demonstrate the ability to utilize coping skills when she is upset.
